Abstract

Hoeveel is een mensenleven waard? Een ongemakkelijke vraag die we liever niet stellen en die wellicht wrevel oproept. De waarde van het leven valt toch niet in geld uit te drukken? Getuigt zo’n vraag niet van gevoelloosheid en gebrek aan ethisch besef? Toch dringt deze kwestie zich steeds meer op aan artsen, verzekeraars, managers en budgetbeheerders in de gezondheidszorg, ambtenaren, politici en burgers. De voortgang van de biomedische kennis en technologie vergroot de behandelingsmogelijkheden: weliswaar is genezing niet altijd mogelijk, maar veel ziekten zijn min of meer beheersbaar geworden en ook kan het leven van ongeneeslijk zieken vaak worden opgerekt. Nog nooit werden wij zo oud en leefden wij zo lang in goede gezondheid. Deze verworvenheid heeft echter een prijskaartje: de gezondheidszorg slokt een steeds groter deel van het bruto nationaal product op. In de Westerse wereld is de financiering van de gezondheidszorg – afgezien van eigen bijdragen (eigen risico) en behandelingen die patiënten uit vrije wil deels of geheel zelf betalen – gebaseerd op collectieve solidariteit, het grondbeginsel van de verzorgingsstaat. Dat wil zeggen, gezonde burgers, werkgevers en de overheid dragen door middel van verzekeringspremies, loonheffingen en belastingen bij aan de kosten van de behandeling van en zorg voor patiënten, die deze kosten niet alleen zouden kunnen dragen. Vrijwel iedereen wil bij ziekte uiteraard zo goed mogelijk behandeld worden en verwacht dat alles wordt gedaan om hem of haar te genezen of in elk geval zijn of haar leven zo lang mogelijk te bewaren – afgezien van de keuze voor actieve levensbeëindiging als het leven een ondraaglijke last is geworden. De kwestie die zich hierbij onvermijdelijk aandient, is die van de sociale rechtvaardigheid: hoe worden schaarse middelen op een eerlijke en billijke manier verdeeld? Wie kan aanspraak maken op welke behandeling? Op hoeveel medische zorg mogen we rekenen en hoeveel mag die kosten? Hoeveel geld, dat wil zeggen premie- en belastingbetalingen, hebben we over en moeten we volgens verzekeraars en overheid over hebben voor de gezondheidszorg? Dergelijke vragen zijn niet op basis van objectieve wetenschappelijke criteria te beantwoorden. Hier zijn waardeoordelen in het geding en deze behoren tot het terrein van de ethiek en politiek. En omdat wij tegenwoordig, althans in de Westerse wereld, in democratieën leven waarin burgers mondig zijn en een stem hebben in de politiek, gaat deze problematiek ons allen aan, ongeacht of we gezond of ziek zijn. De politieke dimensie van gezondheid en ziekte is niet nieuw, maar kent een lange geschiedenis, die nauw samenhangt met de modernisering (industrialisering, democratisering en secularisering) van de Westerse samenleving vanaf het midden van de achttiende eeuw. Dit artikel schetst in grote lijnen de ontwikkeling van het verband tussen enerzijds gezondheid en ziekte en anderzijds de politiek, in het bijzonder de rol van de staat en van burgerschap, en stipt enkele daarmee verbonden dilemma’s aan.

Abstract

Since its emergence as a branch of medicine in the wake of the Enlightenment and French Revolution, psychiatry has experienced significant transformations against the background of different socio-economic and political changes in Western societies. In this wider context we see a recurring tension between the interest of the individual and that of the social body as a whole. This friction is closely related to opposing dynamics in psychiatry and mental health care: humanization versus disciplining, emancipation versus coercion, inclusion versus exclusion, and democratic citizenship versus political subjection. This article provides a conceptual analysis and an historical overview on the ambivalent relations between on the one hand psychiatry and mental health care and on the other politics, and, more particularly, the development of the modern understanding of citizenship.

Abstract

Health has been defined as a basic human right and also, in most of the welfare states in the Western as well as in the former communist world, as a civil right. The Preamble to the Constitution of the World Health Organization drafted in 1945, states that ‘[t]he enjoyment of the highest attainable standard of health is one of the fundamental rights of every human being without distinction of race, religion, political belief, economic or social condition.’ And article 25 of the Universal Declaration of Human Rights, proclaimed by the United Nations in 1948, reads: ‘Everyone has the right to a standard of living adequate for the health and well-being of himself and of his family, including […] medical care.’ No matter whether such rights are explicitly laid down in constitutions, in other (social security) laws, or in the administrative regulations of the welfare state, in many countries health and disease have become an affair of the state as well as an important constituent in the exercise of democratic citizenship. The welfare state provides medical care for individuals – at least for its residents – according to their needs, not because of who they are or what they possess, but because they are citizens with equal rights. Access to health care is an aspect of democratic citizenship: it is considered, not just as a favor or a commodity, but as a right. In fact such rights and provisions reflect long-term historical developments: next to poverty, (ill) health was one of the first social issue targeted by the emerging intervention state in the nineteenth century. As part of the discourse of human rights in liberal-democratic political thought, the notion of health as a right can be traced back to natural law in Enlightenment philosophy and the principles of the American Declaration of Independence (1776) and the Déclaration des droits de l’homme et du citoyen, proclaimed by the revolutionary French National Assembly in 1789. In the course of the nineteenth and twentieth centuries, medicine and politics became mutually entwined to an increasing extent. Health care has become a substantive and increasingly visible issue in democratic as well as authoritarian and totalitarian politics. Most national health care systems in the West, although organized in different ways, epitomize the principle that all inhabitants have an equal right to health care according to need and that its costs are shared equitably. Apart from inevitable biological distinctions between individuals, everybody should have equal opportunities for health. The relation between health and citizenship, however, is far from self-evident and uncontested; it is fraught with complications and ambiguities. The idea that medical care is a human and civil right is easily formulated in the abstract, but it runs into difficulties as soon as practical implementation is at stake. Health cannot be construed as an absolute, legally enforceable right, like freedom of speech or religion, universal suffrage or fair trial. A human or civil right to health cannot be guaranteed for the simple reason that, in spite of the considerable advances of modern medical science and technology, individual health and disease are still, to a large extent, a matter of nature and fate. Governments can at best provide the conditions that put individuals on a more or less equal footing with regard to chances for health. However, as long as the necessary means, such as money, medical knowledge, and an adequate health care infrastructure are lacking, this right will be an illusory promise. The realization of health as a human right has to be fought for and requires resources, solidarity, social responsibility, a long-term perspective, and the effort to resolve difficult questions and conflicts of interests.

Abstract

As a product of nineteenth-century bourgeois society, psychiatry developed in a dynamic between social-political integration and exclusion. Into the twentieth century, institutional psychiatry fulfilled two functions: a medical one (care and cure), which gave priority to the interests of patients, and a social-political one (segregation), which was geared toward freeing society of the nuisance and danger associated with the insane. Which function was most prominent varied with a country’s political constellation. From around 1840 various West-European countries adopted legal regulations for the institutionalization of the insane. Within the margins of the constitutional state, they served to protect citizens against random deprivation of freedom and to allow for effective admission procedures to ensure public order as well as medical treatment for mental patients. The hospitalized insane fell under special jurisdiction and state supervision, which implied a suspension of their civic rights. The liberal contract society assumed autonomous individuals who were capable of serving their own rights and interests while respecting those of others. Liberalism linked citizenship to rationality, autonomy, and responsibility, which were precisely the qualities mental patients had to do without. Mental incapacity counted in fact as the opposite of citizenship as it had been articulated on the basis of the ideals of freedom and equality since the American and French Revolutions. The relationship between institutional psychiatry and liberal-democratic citizenship was ‘negative’ or ‘exclusive’ in the sense that hospitalization in an asylum generally required legal certification and therefore the loss of basic civil rights. In the course of the twentieth century, however, a more ‘positive’ or ‘inclusive’ connection between psychiatry and citizenship was established in two ways. Firstly, the last three decades of the last century saw a growing recognition of the civil rights of the mentally ill, reflecting a shift from values associated with maintaining law and order to values associated with mental patients’ autonomy and consent. Secondly, from the early twentieth century on, in psychiatry as well as in the broader field of mental health care, psychological definitions of citizenship were advanced. Expressing views about the position of individuals in modern society and their possibilities for self-development, psychiatrists and other mental health workers connected mental health to ideals of democratic citizenship. Thus, they were clearly involved in the liberal-democratic project of promoting not only productive, responsible and adaptive citizens, but also autonomous, self-conscious, and emancipated individuals. In this article I focus on the development of mental health care in the Netherlands from the late nineteenth to the early twenty-first century in order to explore its relation to socio-political modernization in general and changing meanings of citizenship in particular. Citizenship took on a broad meaning, not just in terms of political rights and duties, but also in the context of material, social, psychological, and moral conditions that individuals should meet in order to develop themselves and be able to act according to those rights and duties in a responsible way. On the basis of the four different ideals of self-development that I identify, my account is divided into four periods: 1870-1945 (self-development through social adaptation), 1945-1965 (guided self-development), 1965-1985 (spontaneous self-development), and 1985-2005 (autonomous self-development). In the last section I will elaborate some more general characteristics of Dutch mental health care in its socio-political context.

Abstract

After World War II, Dutch psychiatrists and other mental health care professionals articulated ideals of democratic citizenship. Framed in terms of self-development, citizenship took on a broad meaning, not just in terms of political rights and obligations, but also in the context of material, social, psychological and moral conditions that individuals should meet in order to develop themselves and be able to act according to those rights and obligations in a responsible way. In the post-war period of reconstruction (1945-1965), as well as between 1965 and 1985, the link between mental health and ideals of citizenship was colored by the public memory of World War II and the German occupation, albeit in completely different, even opposite ways. The memory of the war, and especially the public consideration of its victims, changed drastically in the mid-1960s, and the mental health sector played a crucial role in bringing this change about. The widespread attention to the mental effects of the war that surfaced in the late 1960s after a period of twenty years of public silence should be seen against the backdrop of the combination of democratization and the emancipation of emotions.

Abstract

This chapter discusses how, in the Netherlands from around 1900 until the mid-1980s, the idea of ‘citizenship’ acquired new definitions in the context of developing ‘mental hygiene’ and outpatient mental health care. Formulating views about the position of individuals in modern society and their potential for self-development, psychiatrists and other mental health workers linked mental health with ideals of democratic citizenship. Thus, they were involved in the liberal-democratic project of promoting not only productive, responsible, and adaptive citizens, but also autonomous, self-conscious, and emancipated members of an open society.

Abstract

Het thema van dit hoofdstuk is de geschiedenis van het verband tussen gezondheidszorg en democratisch burgerschap. Twee aspecten, enerzijds de toenemende invloed van de staat in de samenleving op basis van het idee van sociale maakbaarheid met behulp van professionele deskundigheid en anderzijds democratische burgerrechten en -plichten, vormen het spanningsveld dat centraal staat in mijn betoog. Dat hier sprake is van een spanningsveld komt tot uiting in de vraagstukken die zich in de afgelopen twee eeuwen steeds weer opdrongen als het ging om de collectieve verantwoordelijkheid voor gezondheid en ziekte. In hoeverre is de staat verantwoordelijk voor de gezondheid van burgers en in hoeverre mag de staat zich bemoeien met het persoonlijke leven van burgers om de volksgezondheid te bevorderen? In hoeverre zijn burgers zelf verantwoordelijk voor hun gezondheid en die van hun medeburgers? Wat betekenen burgerrechten in dit verband? Het recht om van staatsbemoeienis verschoond te blijven of het recht op collectief gegarandeerde gezondheidszorg? Kunnen gezondheid en ziektepreventie beschouwd worden als burgerrecht en kunnen deze ook als plicht aan burgers worden opgelegd? Is gezondheid een voorwaarde voor de verwerkelijking van actief democratisch burgerschap en in hoeverre is democratisch burgerschap de voorwaarde voor gezondheid? Hoe verhouden democratisch burgerschap en medisch professionalisme, dat op zichzelf niet democratisch is, zich tot elkaar? Dit zijn normatieve vragen en de mogelijke antwoorden daarop variëren naar tijd en plaats en ook naar politieke en ideologische overtuiging. Door middel van een historische contextualisering van dergelijke vragen en antwoorden, wil ik laten zien dat de tegenwoordige discussies over deze problematiek niet nieuw zijn en zo’n twee eeuwen teruggaan.

Abstract

De psychiatrie kwam in de afgelopen twee eeuwen tot ontplooiing in het spanningsveld tussen humanisering en disciplinering en tussen sociale integratie en uitsluiting. Tot in de twintigste eeuw vervulde de gestichtspsychiatrie twee functies: een medisch-humanitaire (verzorging, bescherming en behandeling), waarbij het belang van de individuele patiënt (en diens naasten) voorop stond, en een sociaal-politieke (segregatie), waarbij het erom ging de naaste omgeving en de samenleving te vrijwaren van overlast en/of gevaar. De liberale rechtsstaat, die de individuele burgers tegen willekeurige vrijheidsberoving beschermde, stelde grenzen aan de mogelijkheid om mensen onder dwang in een gesticht op te nemen. De relatie tussen de inrichtingspsychiatrie en burgerschap was in de negentiende en tot ver in de twintigste eeuw overwegend ‘negatief’ of ‘exclusief’ in die zin dat opname gepaard ging met een juridische procedure (rechterlijke machtiging) of bestuurlijke maatregel (inbewaringstelling), waardoor de patiënt zijn of haar burgerrechten voor de duur van de opname of soms zelfs langer verloor. In de loop van de vorige eeuw ontstond echter ook een meer ‘positieve’ of ‘inclusieve’ verbinding tussen psychiatrie en democratisch burgerschap. Ten eerste kwam er in de jaren zeventig onder invloed van de kritische psychiatrie en de patiëntenbeweging aandacht voor en erkenning van de rechten van de psychiatrische patiënt en werd deze meer en meer als mondig burger met rechten gezien. Ten tweede valt er vanaf de jaren twintig in de psychiatrie en het bredere gebied van de beweging voor geestelijke volksgezondheid en (ambulante) geestelijke gezondheidszorg een tendens waar te nemen om geestelijke gezondheid te definiëren in termen die aanhaakten bij de invulling die aan democratisch burgerschap werd gegeven. In hun beschouwingen over geestelijke volksgezondheid lieten psychiaters en andere aanhangers van het psychohygiënische gedachtegoed hun licht schijnen op de positie van het individu in de moderne maatschappij en diens mogelijkheden om zich daarin te ontplooien. Op deze manier waren zij nauw betrokken bij het veel bredere sociaal-politieke streven naar goed burgerschap. De modernisering van de Nederlandse samenleving en veranderende opvattingen over democratisch burgerschap vormden de steeds terugkerende referentiekaders in het streven naar geestelijke volksgezondheid. Burgerschap kreeg in de loop van de vorige eeuw in Nederland een brede betekenis kreeg, niet louter formeel in termen van politieke rechten en plichten, maar ook substantieel in de zin van de realisering van de nodige materiële, sociale, psychologische en morele voorwaarden om daadwerkelijke uitoefening van die rechten en plichten mogelijk te maken. Het ging daarbij steeds meer om de juiste mentaliteit en attitude. Juist in een egalitaire en democratische maatschappij, waarin onderschikking, dwang en geweld steeds meer naar de achtergrond verdwenen om plaats te maken voor vrijwillige instemming met en actieve deelname aan het sociaal-politieke bestel, werden de mentaliteit, motivatie en geestelijke vermogens van burgers belangrijker. Een democratische orde en civil society konden immers alleen bestaan en in stand blijven als individuen hun vrijheden en rechten op een verantwoorde en beheerste manier gebruikten. Daarbij kwam het aan op internalisering van democratische waarden en persoonlijkheidsvorming. Op basis van verschillende idealen van individuele zelfontplooiing – zelfontplooiing door middel van onvoorwaardelijke sociale aanpassing, geleide zelfontplooiing, spontane zelfontplooiing en autonome zelfontplooiing – zijn verschillende fasen te onderscheiden met betrekking tot het verband tussen de invulling die geestelijke volksgezondheid kreeg en burgerschapsidealen. De idealen van spontane zelfontplooiing en autonome zelfontplooiing brachten een sterk psychologiserende invulling van burgerschap met zich mee. Deze weerspiegelde een tendens in de samenleving als geheel: in het kielzog van de democratiserende sociale verhoudingen raakten de omgangsvormen tussen mensen en de wijze waarop zij zichzelf en elkaar beschouwden in toenemende mate gepsychologiseerd.

In the nineteenth and early twentieth centuries, the relationship between institutional psychiatry and citizenship was “negative” or “exclusive” in the sense that hospitalization in a mental asylum generally implied legal certification and therefore the loss of and potential serious infringement on basic civil rights. In the course of the twentieth century, however, in two ways a more “positive” or “inclusive” connection between psychiatry and liberal-democratic citizenship was established. Firstly, the last three decades of the last century saw a growing attention for and recognition of the civil rights of the mentally ill. In many Western countries the legislation on insanity was amended, reflecting a shift from values associated with maintaining law and order to values associated with mental patients’ autonomy, responsibility, and consent, as well as their right to adequate care and treatment. Secondly, from the early twentieth century on, in psychiatry as well as in the broader field of mental hygiene and mental health care, psychological definitions of citizenship were advanced. Expressing views about the position of individuals in modern society and their possibilities for self-development, psychiatrists, psycho-hygienists, and other mental health workers connected mental health to ideals of democratic citizenship and civic virtue. Thus, they were clearly involved in the modern liberal-democratic project of promoting not only virtuous, productive, responsible and adaptive citizens, but also autonomous, self-conscious, assertive, and emancipated individuals as members of an open society. This article is about the development of mental hygiene and mental health care in the Netherlands after the Second World War in order to explore its relation to the social and political modernization in general and the changing meanings of citizenship and civic virtue in particular. In the course of the previous century, citizenship in the Netherlands took on a broad meaning, not just in terms of political rights and duties, but also in the context of material, social, psychological, and moral conditions that individuals should meet in order to develop themselves and be able to act according to those rights and duties in a responsible way. Notions such as fairness, social justice, social responsibility, tolerance, emancipation and personal development became elements of the definition of good citizenship. On the basis of the three different ideals of individual self-development that I identify, my account is divided into three periods: 1945-1965 (guided self-development), 1965-1985 (spontaneous self-development), and 1985-2005 (autonomous self-development). Before turning to the post-war period, I will briefly sketch the rise of the mental movement in the Netherlands and its socio-political background during the first half of the twentieth century. In the conclusion I will elaborate some more general characteristics of Dutch mental health care in its sociopolitical context.

Abstract

Ondanks haar oriëntatie op de biomedische wetenschappen en psychologie, waarin de oorzaak van geestesstoornissen in het lichaam of de psyché werden gezocht, kende de psychiatrie in de negentiende en twintigste eeuw een sociaal-morele onderstroom. Deze kwam in het bijzonder tot uiting in cultuurkritische reacties op de moderne maatschappij. Psychiaters wezen rond 1900 niet alleen op de biologische oorzaken van waanzin en nerveuze aandoeningen, zoals erfelijkheid, maar zij schonken ook uitgebreid aandacht aan schadelijke gedragingen en sociaal-culturele invloeden die het geestelijk evenwicht op de proef stelden: pauperisme, gebrekkig hygiëne, zedeloosheid, overmatige consumptie van genotmiddelen, seksuele excessen, een slechte opvoeding, de groeiende discrepantie tussen de intellectuele en morele ontwikkeling van de mens, vrouwenemancipatie, de verhevigde strijd om het bestaan, het hectische levenstempo in de steden, nauwelijks bij te houden technologische vernieuwingen en de snelle transport- en communicatiemiddelen. Om de aantasting van geest en zenuwen te weerstaan, waren zelfbeheersing, wilskracht, plichtsbesef en orde en regelmaat geboden. In Nederland leidde de cultuurkritische en sociaal-hygiënische bewogenheid onder psychiaters in het midden van de jaren twintig tot de beweging voor geestelijke volksgezondheid. Naar het voorbeeld van de succesvolle bestrijding van besmettelijke volksziekten bepleitten deze psychohygiënisten preventieve maatregelen om de vermeende toename van geestes- en zenuwziekte in te dammen, bijvoorbeeld door middel van de vroegtijdige behandeling van lichte psychische aandoeningen. Het werkveld dat zij claimden, strekte zich uit van huwelijk, gezin, seksualiteit, voortplanting en opvoeding tot onderwijs, vrijetijdsbesteding, alcoholisme en criminaliteit. Bevreesd voor culturele vervlakking en sociale desintegratie benadrukten zij evenals andere intellectuelen in het interbellum het belang van geestelijke waarden en gemeenschapszin. Het psychohygiënische gedachtegoed sloot aan bij het beschavingsoffensief, dat in de negentiende eeuw vooral uitging van de liberale burgerij, maar dat vanaf de eeuwwisseling vervlochten was geraakt met het confessionele en sociaaldemocratische emancipatiestreven. Naarmate de democratisering voortschreed, achtten vertegenwoordigers van de elites het des te noodzakelijker om het volk zedelijk te verheffen. Voor verantwoord burgerschap waren zelfbeheersing en gemeenschapszin vereist. Volksopvoeding onder leiding van de culturele en wetenschappelijke elite gold als hét middel om een samenbindende morele oriëntatie op burgerlijke fatsoensnormen en algemene christelijke waarden te garanderen. Met hun normatieve invulling van het begrip geestelijke volksgezondheid sloten de leidende psychohygiënisten nauw aan bij het streven naar een goed geordende massasamenleving door middel van de onvoorwaardelijke aanpassing en onderschikking van het individu aan een collectief gedeeld normen- en waardepatroon. De psychohygiëne bood psychiaters een kader om zich op te werpen als hoeders van de publieke moraal. De basis daarvoor werd al voor de Tweede Wereldoorlog gelegd, maar in de periode 1950-1980 manifesteerden enkelen zich nog sterker als morele entrepreneurs in de Nederlandse samenleving. Daarbij maakte hun defensieve houding ten aanzien van moderniseringsverschijnselen al gauw plaats voor vernieuwingsgezindheid. Dit artikel gaat over de bijdragen van psychiaters aan de taboedoorbrekende morele heroriëntatie ten aanzien van een aantal gevoelige kwesties, die het persoonlijke leven van veel Nederlanders direct raakten: godsdienst, seksualiteit, geboorteregeling, abortus, euthanasie, drugsgebruik en de verwerking van de oorlog. Daarbij zal ik aangeven wat de nieuwe – in hoge mate psychologiserende – publieke moraal inhield en waarom deze vanaf de jaren tachtig aan betekenis inboette.
